Serious Case Reviews and Child Protection Inquiries

Individual Child Protection in the Community inquiries involving with Local Authorities/Health probation having prior involvement.

[Updated 14 mar 2016 for Lucy Gates, but not for others, follow links to check if report has been released. If someone could update list it would be appreciated]

This list was first posted on my post on Maria Colwell [14]. However it has proved so popular that I have given it a post of its own so that it is more easily readable.

Cases nowadays have to undergo a serious case review. Serious case reviews (SCRs) are undertaken by local safeguarding children boards (LSCBs) for every case where abuse or neglect is known or suspected and either a child dies, or a child has been seriously harmed and the case gives rise to concerns about the way in which local professionals and services worked together to safeguard and promote the welfare of children.[Dept for Education][12]. They are usually published on line.

However before approximately year 2000 there are approximately 50 reports , most of which have not been published. If anyone has the time to request any of these on the Whatdotheyknow [1] website many would be grateful.

It is just a matter of doing a bit of research into the exact name of the report if available, and the exact up to date authority to ask. Most details are available below. If anyone would like any help please contact me on
